The following documents and authorizations are required prior to your first visit: 1. Patient Information Form (two pages): can be downloaded and should be filled out prior to coming to the office. You can be fax or bring it to your initial session. Please sign both pages. 2. Insurance Card A copy of both the front and back of your card. 3. Authorization Number: If your insurance company requires an Authorization Number (contact your insurance company to see if this is needed), please obtain this and provide it to me at your session. If you do not obtain this authorization, you are responsible for full payment of your session. 4. Copay: Contact your insurance company for amount. 5. Deductible. If you have a deductible you are responsible for the payment of your session until your deductible is met. (This is important to clarify with your insurance company prior to your session).
